I have been asked to provide a diagnostic assessment for dyslexia. It would be appreciated if you, or another member of staff who knows the young person well, could complete as much information as possible on the form below.
As part of the process access arrangements may be required. I would be happy to complete the Part 2 of the JCQ’s Form 8 to support an application for these requirements. If this service is necessary, it comes at no extra cost to the school or parent. Please could you supply me with a copy of Form 8, Part 1 which the JCQ stipulates must be provided to the assessor prior to the assessment – this can be sent electronically to hello@dyslexiasolutions.co.uk or to the postal address above.
I can provide scans of my APC (Assessment Practising Certificate) by email for your records. Please do not hesitate to contact me should you require any further information.
